Aged Care Strategy Unit

Aged Care Strategy staff work with the South Eastern Sydney Local Health District (SESLHD) community aged-care service providers to influence practice, shape service delivery, identify need and risk, support sustainability of effective services and facilitate contract compliance and reporting.

The Aged Care Strategy Unit provides strategic leadership and support to the following Commonwealth and NSW Health funded programs within SESLHD:

Commonwealth Aged Care Assessment Services:

  • Aged Care Assessment Program (ACAP)
  • Regional Assessment Service (RAS)

Commonwealth Aged Care Community Services:

  • Commonwealth Home Support Programme (CHSP)
  • Transitional Aged Care Programme (TACP)

NSW Health Community Services:

  • ComPacks Program

My Aged Care

The national My Aged Care system has been established by the Federal Government as the central point for access to an aged care assessment and Commonwealth funded aged care services. My Aged Care comprises a national contact phone number 1800 200 422 and a website with information about aged care services www.myagedcare.gov.au.

Referring to Aged Care Services

All referrals for Commonwealth funded aged care assessment services and community programs must be directed to My Aged Care by phoning 1800 200 422 or by making an online referral through the My Aged Care website www.myagedcare.gov.au
