The SES HealthPathways website aims to improve the management of patients in and between primary and secondary care, through clearly defined pathways and protocols, as well as improving communication between clinicians.
It is available to local health and social care service providers including local General Practitioners, Allied Health Professionals, residential aged care facilities, local government and non-government organisations, and South Eastern Sydney Local Health District clinical staff. On 5th February 2019 the team celebrated reaching the milestone of 100 pathways just five months after their first 50 pathways went live.
