Acute Care Team (ACT)

What we do?

Community mental health service conducts urgent community-based assessment and short-term treatment interventions for people with a mental illness in crisis. We also provide intensive support for people with prolonged and severe mental illness and associated high-level disability, non-urgent, and continuing-care services for people with a mental illness and their families or carers in the community.

What to expect?

Our service uses an assertive outreach approach that may result in clinician being involved with care for extended periods of time or providing more episodic care. A clinician can help with managing mental health declines, cope with distress and ways to improve mental health and wellbeing.
