Community Management Centre Honoured with 2025 Excellence in Aboriginal Healthcare Award

The South Eastern Sydney Local Health District’s (SESLHD) Community Management Centre (CMC) Emergency Department (ED) Follow-Up Service has won the Excellence in Aboriginal Healthcare Award at the 2025 CORE Awards for its outstanding contribution to culturally safe and responsive care. 

The award recognises the CMC’s Aboriginal Health ED Follow-Up Service, an initiative  which was pioneered by Aboriginal health workers, and is a collaborative effort between the Prince of Wales Hospital, the Aboriginal Health Directorate, Strategy Improvement and Innovation, and the Virtual Health Care teams.  

The program reflects the principles of the Healthy Mob, Strong Community: SESLHD Aboriginal Health Plan 2024-2026 in action, through aiming to improve health outcomes for Aboriginal patients across the region. 

The CMC operates as a district-wide service, working alongside existing local providers to deliver virtually enabled care that complements, rather than replaces, traditional face-to-face or specialist clinical services. 

The Aboriginal Health ED Follow-Up Service supports Aboriginal patients aged 16 and over who have been discharged from a SESLHD ED, did not wait to be seen, or left against medical advice. The service facilitates timely connection with Aboriginal health professionals, support networks, and Community organisations, helping to reduce re-presentations to ED and strengthen care pathways. 

Crucially, the program also collects patient feedback to identify and address systemic barriers and enhance cultural safety within emergency care settings. 

The award highlights the success of the CMC’s innovative approach in fostering equitable, Community-driven health care for Aboriginal people across South Eastern Sydney.
