Respiratory Coordinated Care Program
Respiratory Coordinated Care Program (RCCP) / Community Respiratory Service
The Respiratory Coordinated Care Program (RCCP) is a hospital-based community program implemented across both St George and Sutherland Hospitals that provides home visits to patients who are unwell from a respiratory condition. This specialised hospital-based community program is also designed to assist people with advanced lung disease to live optimally well in their homes.
Our multidisciplinary team includes Respiratory and Supportive Care doctors, nurses, pharmacists and physiotherapists who work together to provide exceptional, specialised respiratory care, both short and long term, to patients in the community. This has proven to decrease the number of visits to hospital and reduce length of stay, allowing patients to remain optimally well at home with the added support of our 7 day a week service. In addition, we also serve as a vital link between the hospital and the patient, and/or the patient and their GP, ensuring seamless communication and coordinated, comprehensive care throughout their treatment journey.
We are contactable between the hours of 0800-1630, 7 days a week
