RCCP: Information for patients and families

How Can We Help?

The Respiratory Coordinated Care Program (RCCP) is a specialised hospital-based community service operating 7 days a week (0800–1630).

We provide home visits to support people living with both acute and chronic respiratory conditions, including:

  • Chronic Obstructive Pulmonary Disease (COPD)
  • Interstitial Lung Disease (ILD)
  • Bronchiectasis
  • Chest infections

Our program is also designed to help people with advanced lung disease live optimally well at home, safely and with the right supports.

The RCCP reflects our hospital’s commitment to health equity, prevention, and early intervention.

We aim to:

  • Reduce hospital admissions and readmissions
  • Shorten hospital length of stay (HLOS)
  • Support self-management of your respiratory condition
  • Improve your quality of life at home

Research has shown the RCCP approach improves outcomes for people with COPD and reduces reliance on hospital-based care (Spiliopoulos, Donoghue, Clark, & Dunford, 2008).

Our care is safe, compassionate, and integrated:

  • Respiratory monitoring at home
  • Breathlessness management
  • Sputum clearance education
  • Mobility and home safety support
  • Oxygen and non-invasive ventilation support
  • Inhaler technique and education
  • Linking you to other helpful services

We ensure care is provided in the right place, at the right time, with seamless communication between hospitals, GPs, and families.
 

The RCCP team is multidisciplinary and collaborative, bringing together:

  • Respiratory & Supportive Care doctors
  • Nurses
  • Respiratory physiotherapists

We work together to provide integrated, evidence-based care, and our service culture promotes teamwork, innovation, and the wellbeing of both staff and patients.

You may be eligible if you:

  • Live in the Bayside or Georges River Council catchment areas
  • Have an underlying respiratory illness

Referrals can be made by your local GP or respiratory specialist.
