Respiratory Coordinated Care Program (RCCP)
The Respiratory Coordinated Care Program (RCCP) is a hospital-based community service that supports people with acute and chronic respiratory conditions. These include Chronic Obstructive Pulmonary Disease (COPD), Interstitial Lung Disease (ILD), bronchiectasis, and chest infections. The RCCP helps people to stay healthy and remain safely in their own homes.
The RCCP service can assist with respiratory monitoring at home, breathlessness management, sputum clearance education, mobility and safety at home, managing home oxygen and non-invasive ventilation, inhaler education, and facilitating referrals or linking to relevant services. The service operates seven days a week.
