Referral information for GPs and Medical Officers
Our hospital has a centralised Adult Outpatient Department as well as separately managed outpatient clinics.
We provide outpatients clinics across a range of surgical and medical specialties. Patients can be referred by the Emergency Department, a GP or a Specialist. Find the clinic and referral guidelines for your patient by visiting the clinic or department webpage below:

Our Hospital is piloting electronic referrals (eReferrals) with some of our outpatient services from November 2023. Referrers are encouraged to send eReferrals where they are available. The Central Eastern Sydney Primary Health Network also has a directory of public and private specialists in the local area.
We cannot accept your referral without the mandatory information below. Please review individual Department webpages for further details.
Your patients cannot be triaged until all mandatory information is received. In the interest of patient safety, please ensure you have fulfilled these requirements to prevent longer wait times than necessary. Using our referral form to ensure this criteria is appropriately addressed. All information outlined in red must be completed. Your referral will not be accepted without this information.
Mandatory information on all referrals includes:
- Patient details, including full name, date of birth, address, mobile phone number and Medicare number at minimum
- The name of one Specialist Consultant being referred to (do not specify more than one Consultant)
- The name of the clinic being referred to
- Next of Kin’s name and phone number
- Reason for referral and diagnosis
- Clinical urgency.
