Community Diabetes (ComDiab)
This program supports people with diabetes. We provide practical information about healthy food choices, being active, and preventing complications. It supplements the education and care provided by your doctor.

- information in multiple languages
- tailored programs to meet individual needs
Who we support
This program is for:
- adults with uncomplicated Type 2 diabetes, post-gestational diabetes, or pre-diabetes
- families and carers of people with these conditions

You can get a referral from your doctor or contact us directly.
