South East Aboriginal Health Care (SEAHC) - Integrated Team Care
This service is for Aboriginal and Torres Strait Islander people living in the South Eastern Sydney Local Health District (SESLHD) who have ongoing health challenges. This area is the traditional land of the Dharawal, Gadigal, Wangal, Gweagal, and Bidjigal peoples.

You will receive support from:
- Aboriginal Outreach Worker
- Care Coordinator (Health Clinician)
What we do
We support mob to tackle their ongoing health challenges like high sugar (diabetes), heart disease, stroke, and breathing issues (asthma, lung disease). We offer holistic and culturally appropriate health care. We assist you in managing your health.
We can support you with your GP Chronic Condition Management Plan, which might include:
- Working with specialists and other health professionals
- Creating strategies to monitor and maintain your health
- Supporting you with appointments
- Connecting you to services
- Providing access to approved medical aids
