• To provide leadership and direction in the development, implementation and evaluation of the Aged Care & Rehabilitation Clinical Services Plan and service gaps.
• To provide opportunity for discussion, advice and comment to the District Executive and Ministry of Health on relevant issues related to the stream.
• To provide input and direction into the development of relevant District-wide procedures and models of care.
• To facilitate communication of Federal, State and District initiatives related to the stream.
• To promote education and research into relevant areas and facilitate the sharing of service experience.

• Advance Care Planning
  • Normalise routine use of end of life/ACP tools; support staff to effectively undertake conversations;  ensure access to the documentation ​
  • Strategy: clinical re-design project – collaboration with Palliative Care and Clinical Governance, commencing Feb 2023.​
• Shared care models
  • Aged care with peri-operative and post-operative management of the older person; and ​
  • Rehabilitation with cancer survivorship; stroke; and chronic disease​​
• Frailty pathway
  • District approach, link existing services and gap analysis​
