Emergency Department to Community (EDC)

This program supports people who often visit the Emergency Department (ED) due to complex health and social needs. The goal is to provide support in the community to reduce unnecessary visits to the ED. 

Our Team

You will receive support from: 

• Registered Nurses
• Social Workers

EDC clinicians work across all the EDs in the South Eastern Sydney Local Health District (SESLHD).

What we do

• We help people manage their health in the community
• We connect people with community services and resources

Who we support 

We help people between the ages of 18 and 65 who often present to the ED and have chronic health conditions.

How to get our support

If you are eligible, we will contact you after you leave the hospital.
