Out of Hospital Care (OHC) Program
This program supports clients discharged from the hospital with non-clinical care at home for the short or medium term.
Our Team
The SESLHD OHC Coordinator and OHC Relationship Manager ensure the effective management and implementation of OHC packages.
What we do
There are three types of OHC packages in the program:
- Community Packages (ComPacks)
- Safe and Supported at Home (SASH)
- End of Life (EoL)
Who we support
This service is for clients who have been in the hospital or are receiving outpatient or community health services.
How to get our support
Only health staff can make referrals to this service.
