Planned Care for Better Health (PCBH) Care Coordination
This is a 12-week program for people with ongoing health problems. We support people to live well in the community. The program focuses on personalised care, promoting wellness, empowerment, and independence. It works with services to provide health coaching, care navigation, and care coordination.

You will receive support from:
- Care Coordinators and Navigators who are either registered nurses or allied health clinicians
What we do
We provide:
- Health Coaching: we encourage you to make positive changes that help you stay healthy
- Care Navigation: we connect you with community services that can help
- Care Coordination: we work with your doctor and other services to make getting help easier
Who we support
We support people over 16 (or over 15 for Aboriginal or Torres Strait Islander people) living in the South Eastern Sydney Local Health District (SESLHD) with ongoing health problems.
How to get our support
If you go to the hospital, you may be automatically referred to this program if you qualify. You may also be referred if you have been in the hospital before and continue to have health problems.
You can refer yourself using the referral form or you can ask a support agency or health professional to refer you.
A Care Coordinator or Navigator will call you to discuss the program and ask if you want to enrol.
