Cancer Survivorship Service - Information for health professionals
The Prince of Wales Hospital Cancer Survivorship Service aims to improve the health and wellbeing of people living with and beyond cancer. We provide multidisciplinary outpatient services to support people in recovering from cancer treatment and attaining healthy lifestyles and general well-being.

Inclusion criteria
Patients must have been treated for any kind of cancer at Nelune Comprehensive Cancer Centre, Prince of Wales Hospital or Royal Hospital for Women. They must have been:
1. Treated with curative intent
OR
2. Patients treated with palliative intent with stable disease and prognosis >2 years, assessed to be suitable for survivorship services by their treating cancer specialist.
Exclusion criteria
Patients with advanced or metastatic cancer, with progressive disease or prognosis <2 years.
Referrals can be made by treating cancer specialists at Prince of Wales Hospital, or by General Practitioners. Please email this completed referral form to SESLHD-NCCC-Referrals@health.nsw.gov.au or fax to 02 9382 5090.
The following information is mandatory for all incoming referrals:
- General patient information (name, contact number, Medicare no. etc.)
- Cancer diagnosis date and details (stage, cancer location and histology)
- Specialist being referred to
- Comorbidities
- Medical history
- Medications
- Findings from investigations
- Current management plan
- Reason for referral / cancer survivorship issues, such as:
- Activity concerns
- Cognitive issues
- Depression/anxiety
- Dietary concerns
- Fatigue
- Fear of cancer recurrence
- Pain
- Peripheral oedema
- Reproductive health (sexual health, fertility, menopausal symptoms)
- Sleep issues
- Social concerns (education, work, financial, relationships)
- Others

Who will contact your patient to schedule an appointment?
Once a referral has been received and triaged, the Nelune Comprehensive Cancer Centre New Bookings Officer will contact the patient to make an appointment.

We are a Medicare service. You must provide a referral that is addressed to one of our Specialist team if your patient wishes to be bulk-billed.
