Rapid Access Cardiac Clinic - Information for Health Professionals

The Rapid Access Cardiac Clinic is a medically led outpatient service located in the Community Assessment Unit, which provides timely access to high-quality cardiology care for low to intermediate risk cardiac patients.

Inclusion criteria

  • Age 16+
  • Resides within the SESLHD catchment area or known to a POWH cardiologist
  • Requires urgent appointment (within 3 days)

Clinical condition:

  • Undifferentiated chest pain without high-risk features
  • New or recurrent atrial fibrillation, atrial flutter and/or palpitations without significant haemodynamic compromise
  • Decompensated heart failure
  • New or recurrent syncope without high-risk features
  • Perioperative outpatients requiring cardiac clearance for surgery

Exclusion criteria

  • Resident of an aged care facility 
  • Existing cardiology appointment within 7 days
  • Moderate to severe breathlessness at rest
  • Any chest pain with high-risk features
  • Febrile illness
  • Acute behavioural disturbance

Prince of Wales Hospital Outpatient Department is using electronic referrals. eReferrals are electronic SmartForms that can be sent directly to the hospital facility from your practice system or the MyHealthLink Portal. An eReferral is an electronic document that can be sent securely, replacing other methods such as faxing or posting paper referrals. The above solutions enable referrals to be sent, received, and managed electronically. Referrers and patients will also receive notification of referral outcomes.

Publicly funded outpatient clinic

The Rapid Access Cardiac Clinic is a publicly funded outpatient service led by a Cardiology Advance Trainee and supervised by our Consultant Cardiologists. As a public clinic, the patient will not be charged personally for medical or hospital services (except for the hire of some equipment).

Private referrals 

If your patient prefers to see a specific cardiologist, they may do so by arranging an appointment at that clinician’s private practice. Please note that it is inappropriate for our administration team to recommend a Consultant for your patient. However, we have listed the names of our Consultants above to help them decide on the most appropriate practitioner if they have a preference to be seen in private practice.

Who will contact your patient to schedule an appointment?

Once the eReferral has been received and accepted a notification will be sent to the referrer and patient. Appointment details will be provided via a phone call to the patient.

Who can you contact to follow up on a referral? 

You may contact the Community Assessment Unit on (02) 9382 9200 to follow up on a referral.

What is the process if a patient misses or cancels their appointment?

If your patient needs to change or cancel their appointment, they should phone the Community Assessment Unit on (02) 9382 9200.
